The Palace at Home is a Medicare certified home health agency (since 1997).
Under the direction of our patients' physicians, our health care professionals provide patients covered under Medicare, the following Nursing Care services:
At the Palace at Home, a liaison of relationship is established with physicians, hospital case managers, discharge officers, trust officers and family members. We coordinate the patient’s overall health care under the direction of your physician, in order to provide continuing nursing care in the comfort of your home. Our case managers and nursing coordinators, coordinate our nurses visits as prescribed by your physician. We are committed to provide nursing services to meet every patients’ schedule, always taking the necessary time to discuss home care needs and alternatives for recovery with our patients and their famililes..
Patient satisfaction is our measure of success. We conduct periodic surveys among our patients through internal written surveys with our patients and their families and also deploy an independent survey agency (Fazzi Associates) to ensure we continuously improve our patient and customer care.
Our goal is to make the home environment clinically safe and the transition from medical facility to home smooth, relaxed and hassle-free for the patient.

In certain cases, assistance of Home Health Aides may be needed in order to support in a variety of personal care tasks for patients covered under Medicare. In order to meet the goals of the care of plan, our RN's (Registered Nurses) or Therapists conduct supervisory visits every 14 days to ensure that our Home Health Aides meet the goals of the plan of care for each individual patient.
